Form 11 has to be filled while joining a company to become a member of the EPF scheme. It is a self -declaration form that helps the employer to find out whether the employee is already a member or not. Also, while joining a new organization, form 11 is to be submitted. This is done to transfer the PF account from one organization to another. Moreover, Form 11 also contains the history of the EPF account of the employee.
Let us consider you have a salary of more than 15000Rs. Also, the employee’s strength of the company exceeds the mark of 20. Then the individual is liable to contribute to the EPF account. Moreover, the employee has to provide details of the EPF account in Form-11, if any.
Who Needs To Fill The Form-11?
An employee must fill form-11 at the time of joining a new establishment. Furthermore, employees switching to a new organization has to fill it too. It displays all the contributions made by the employee previously, if any.

Earlier employees had to fill form 13 to transfer the PF account. But with the modified version of form 11, this can be done through form11 itself.
Form 11 Details To Be Filled
Form 11 contains the following fields that are needed to be provided by the employee:
- Name of the individual
- His/her date of birth
- Father’s or Husband’s name
- Gender of the employee
- His/her personal email-id.
- Mobile number to be registered
- Marital status
- His/her educational details.
- His/her KYC details. This includes PAN card, Aadhaar card, bank account details, etc.
You also need to give in details that we’re related to the past employer ( if you were enrolled in the EPF scheme) :
- Previous employer details.
- Date of joining the organization
- UAN(universal account number ) of the member
- Provident fund ID number of the employee
- Verification of the employee details.
- Scheme certification number, if any.
If the employee is based out of the country, then additional documentation is to be provided. Which include:
- Country of Origin
- Passport Number
- Validity of passport
The Undertaking Of The Employee
Some declaration points are mentioned in form 11 is to be read once. After that, you need to sign in the declaration. Here, you would also have to mention the date and place of signing too. This is to be quoted on the left-hand side of the undertaking.
Declaration By The Current Employer
The present employer is needed to take a necessary declaration. The employer has to fill up the concerned details and sign the same. Then this declaration is to be sealed by the employer. This declaration contains detailed furbished by the employee. This declaration includes:
- Date of joining of the member
- PF id assigned to the employee
- UAN number for EPFO portal provided
- Verification of the KYC details.
Form 11 Submission
Once the above process is complete, the sealed form is submitted to the nearest regional EPFO branch. And the same is updated in the database within a week.
